Items needed before a doctoral degree may be awarded:

·         One original, signed ("wet" signatures) signature page (required)

·         Electronic filing of dissertation (ETD) or supporting document via ProQuest (required)

·         Doctoral Form III (required)

·         UCSB Doctoral Exit Survey and Survey of Earned Doctorates (required)

·         All grades posted (required)
*Exception: degree may be awarded if student registered in 598 (thesis prep) or 599, BUT in order for a grade to be posted, department must submit the Registrar's Office Grade Change Form.

·         Committee Form IA (required only if committee differs from what is on file)

·         Interdisciplinary emphasis check sheet (required only if student added an emphasis)

·         CCUT completion email (required only if student added CCUT)
